Need to add a cancelation reason prompt (input) to events

We need to be able to have users or managers who cancel themselves or their line reports from an event. This is vital for reporting:


If a user cancels themselves or any of their line reports off of an event , we need the LMS to prompt them with a list of cancellation reasons:



This could be:


1. Sickness


2. Personal (e.g Bereavement / Child Care)


3. Operational (Staff Shortage)


4. Booked Holiday


5. Strikes / Undoable to travel


6. Course no longer applies to job role / skill set
