Add more Form Fields to reporting so we can cut down on user error filling in the forms where the info is already in Access Learning

We use the Access Learning Forms as a Call Listening/Auditing system. One really annoying thing - is that the system knows exactly who is creating and filling in the form / and the date created - because it displays it in Info in the performance section - but you can't pull that information through into the actual reporting system - and onwards. That would remove the need for them to type in their name by hand (and hence make mistakes....).
